Regardless of age, when placement in a nursing home or long-term care facility becomes necessary, an Office of Aging Assessor may be assigned. After completing an assessment, the worker may determine that the person can be diverted from placement and remain at home with the provision of an intensive level of home-based services. The Office of Aging may continue to assist seniors even after placement in a long-term care facility through the Ombudsman program. The Long-Term Care Ombudsman is responsible for investigating and resolving complaints from people receiving long-term care services, whether those services are provided in a facility such as a nursing home or in the seniors own residence in the community.
